What Is the Best Injection for Sciatica? Epidural Steroid Injections Explained

If you have sciatica, you may have already tried medications, physical therapy, stretching, and activity modification—and still have pain shooting down your leg.

At that point, you may start wondering:

“Would an injection help?”

And if so:

“Which injection is best for sciatica?”

The answer is not as simple as choosing one injection that works best for everyone.

For patients with sciatica caused by irritation or compression of a spinal nerve, an epidural steroid injection (ESI) can sometimes provide meaningful relief. But there are several different ways to perform an epidural injection, and the approach that makes the most sense depends on the location of the nerve irritation, the underlying cause, your symptoms, your MRI findings, and your overall clinical picture.

As an interventional spine physician, I frequently see patients who arrive with an MRI showing a disc herniation and assume they automatically need an injection. Others have been told they need an epidural without understanding why a particular approach was recommended.

The goal of an injection should not simply be to “put medicine in the spine.”

The goal is to deliver treatment to the right area, for the right diagnosis, in the right patient.

This article explains how epidural steroid injections work, the different types of epidural injections used for sciatica, when they may be helpful, how long relief can last, what the risks are, and how to determine whether an injection is appropriate for you.


What Is an Epidural Steroid Injection?

An epidural steroid injection is a procedure in which a corticosteroid medication, usually combined with a local anesthetic, is placed into the epidural space around the spinal nerves.

The epidural space surrounds the dura, the protective covering around the spinal cord and nerve roots.

The purpose of the injection is to reduce inflammation and irritation around a spinal nerve that may be causing radicular pain.

For sciatica, the irritated nerve is usually a lumbar or sacral nerve root.

What does the steroid actually do?

A herniated disc or narrowed portion of the spine can irritate a nerve in two ways:

  1. Mechanical compression
  2. Chemical inflammation

A nerve can become painful even when the amount of physical compression is relatively modest because inflammatory substances associated with a disc herniation can irritate the nerve root.

The corticosteroid is intended to reduce this inflammatory component.

Importantly, an epidural steroid injection does not physically remove a herniated disc or enlarge a narrowed spinal canal.

Instead, it is intended to reduce the inflammation and pain associated with an irritated nerve.


Does an Epidural Steroid Injection Work for Sciatica?

For appropriately selected patients with lumbar radiculopathy, epidural steroid injections can provide short-term improvement in pain and disability.

A 2025 systematic review from the American Academy of Neurology evaluated 90 randomized controlled trials and concluded that epidural steroid injections probably reduce short-term pain and disability in cervical and lumbar radiculopathies. The evidence for long-term pain relief was insufficient.

This distinction is important.

An epidural injection should generally not be viewed as a permanent cure for sciatica.

Instead, it may provide a period of pain relief that allows a patient to:

  • Walk more comfortably
  • Sleep better
  • Participate in physical therapy
  • Resume normal activities
  • Reduce reliance on pain medications
  • Allow time for an irritated nerve to recover naturally

For some patients, that temporary reduction in pain is all that is needed to get through an acute episode.

For others, an injection may provide little or no meaningful relief.


What Is the Best Injection for Sciatica?

There is no single epidural injection that is best for every patient with sciatica.

The three major approaches are:

  1. Transforaminal epidural steroid injection
  2. Interlaminar epidural steroid injection
  3. Caudal epidural steroid injection

Each approach places medication in the epidural space, but they reach the target area differently.

The choice depends on the patient’s symptoms, imaging findings, anatomy, and clinical diagnosis.


1. Transforaminal Epidural Steroid Injection

A transforaminal epidural steroid injection (TFESI) is performed through the neural foramen—the opening through which the spinal nerve exits the spine.

This approach allows the physician to place medication relatively close to a specific nerve root.

For that reason, a transforaminal injection is often particularly useful when a patient has:

  • Unilateral radicular pain
  • A disc herniation affecting a specific nerve root
  • Foraminal stenosis
  • Lateral recess or foraminal pathology
  • Symptoms that clearly correspond to a particular nerve root

For example, a patient with a left L5-S1 disc herniation compressing the S1 nerve root and producing classic left S1 radicular pain may be an appropriate candidate for a targeted transforaminal injection.

Why choose a transforaminal injection?

The major advantage is targeting.

Rather than simply placing medication into the epidural space without a specific target, the transforaminal approach allows the physician to deliver medication near the affected nerve root.

This is particularly appealing when the patient’s clinical symptoms and MRI findings point toward a specific nerve.

Recent evidence continues to investigate whether the transforaminal approach provides better outcomes than other epidural approaches. A 2026 meta-analysis comparing techniques found greater short- and intermediate-term improvements with transforaminal injections than interlaminar injections in the included studies, although differences between approaches should still be interpreted in the context of individual patient anatomy and diagnosis.

When might a transforaminal injection not be the best choice?

A targeted transforaminal injection may be less appropriate when:

  • Symptoms are not clearly radicular
  • There is no convincing nerve-root target
  • Multiple levels are involved
  • Anatomy makes the approach unfavorable
  • The primary problem is axial low back pain rather than nerve-root pain

2. Interlaminar Epidural Steroid Injection

An interlaminar epidural steroid injection approaches the epidural space from the back of the spine between two adjacent laminae.

This can allow medication to spread within the epidural space and potentially reach one or more irritated nerve roots.

An interlaminar injection may be considered when:

  • Symptoms are less strictly unilateral
  • There is multilevel pathology
  • A broader epidural distribution is desirable
  • The anatomy makes an interlaminar approach preferable
  • The clinical target is not easily reached through a transforaminal approach

The choice between interlaminar and transforaminal injection is therefore not simply a matter of one being “better.”

It is a matter of which approach best fits the patient’s anatomy and clinical problem.


3. Caudal Epidural Steroid Injection

A caudal epidural steroid injection enters the epidural space through the sacral hiatus at the bottom of the sacrum.

The medication then travels upward within the epidural space.

Caudal injections may be particularly useful in certain patients with:

  • Multilevel lumbar pathology
  • Multiple areas of nerve irritation
  • Previous lumbar surgery
  • Altered anatomy that makes other approaches more difficult
  • Situations where a lower-entry epidural approach is preferable

Because the medication enters the epidural space farther away from the affected nerve root, a caudal injection may not provide the same degree of targeted delivery as a transforaminal injection.

However, that does not make it an inferior procedure.

It simply means that the appropriate approach depends on the clinical situation.


So Which Epidural Injection Is Best?

For many patients with one-sided sciatica caused by a disc herniation affecting a specific nerve root, a transforaminal epidural steroid injection is often a particularly logical approach because it allows medication to be delivered close to the affected nerve root.

However, that does not mean transforaminal injections are always superior.

A patient with multilevel disease, previous surgery, different anatomy, bilateral symptoms, or another pattern of pathology may be better suited to an interlaminar or caudal approach.

The best injection is the one that best matches the patient’s diagnosis and anatomy.


Why Does the Injection Need to Be Image-Guided?

Epidural steroid injections are generally performed using imaging guidance, most commonly fluoroscopy.

Fluoroscopy allows the physician to visualize the bony anatomy and confirm needle placement.

Contrast dye is typically used to confirm that the medication is reaching the intended epidural or perineural location and to help identify unintended vascular or other spread.

This is particularly important for targeted injections around spinal nerve roots.

Why isn’t the injection simply done by feel?

Because the epidural space and spinal nerve roots are deep structures that cannot be reliably located based solely on surface anatomy.

Image guidance improves the ability to place the needle accurately and allows the physician to verify the pattern of contrast before medication is injected.


What Happens During an Epidural Steroid Injection?

The procedure typically involves several steps.

1. Positioning

You will usually lie on your stomach on a procedure table.

2. Skin preparation

The skin is cleaned with an antiseptic solution and the injection site is identified.

3. Local anesthetic

A small amount of local anesthetic is injected into the skin and deeper tissues to numb the area.

4. Needle placement

Using fluoroscopic guidance, the physician advances a specialized needle toward the epidural or targeted nerve-root area.

5. Contrast injection

A small amount of contrast material may be injected to confirm appropriate needle placement and contrast flow.

6. Medication injection

The steroid, often combined with a local anesthetic, is then injected.

7. Recovery

After the procedure, you are monitored for a short period before being discharged.

The procedure itself is usually relatively brief.


How Quickly Does an Epidural Injection Work?

The response varies considerably from patient to patient.

Some people experience improvement relatively quickly, particularly from the local anesthetic component.

The steroid’s anti-inflammatory effect may take several days to develop.

It is therefore common for patients to experience:

  • Immediate temporary relief
  • No immediate improvement followed by improvement over several days
  • Partial improvement
  • No meaningful improvement

A lack of immediate relief does not necessarily mean the injection has failed.

Likewise, immediate pain relief from the local anesthetic does not necessarily predict how much longer-term benefit the steroid will provide.


How Long Does an Epidural Steroid Injection Last?

This is one of the most common questions patients ask.

Unfortunately, there is no single answer.

Some patients experience relief for:

  • Several days
  • Several weeks
  • Several months
  • Much longer

Others experience little or no benefit.

The duration of relief depends on factors including:

  • The underlying diagnosis
  • Severity of nerve compression
  • Duration of symptoms
  • Degree of inflammation
  • The specific nerve involved
  • Patient factors
  • Whether the underlying condition is continuing to irritate the nerve

The evidence suggests that the primary benefit of epidural steroid injections is generally short-term rather than reliably long-term pain relief.


Can an Epidural Injection Cure a Herniated Disc?

No.

An epidural steroid injection does not remove a herniated disc.

However, this does not mean that the injection cannot be extremely useful.

Many disc herniations improve naturally over time, and the associated nerve inflammation can settle down.

An epidural injection may reduce pain during this recovery period.

Think of the injection as potentially helping you through the healing process, rather than mechanically fixing the disc.


Can an Epidural Injection Prevent Surgery?

Sometimes an injection may help a patient avoid surgery, but it should not be presented as a guaranteed alternative to surgery.

Some patients with severe radicular pain improve enough after an injection and rehabilitation that surgery is no longer necessary.

Other patients continue to have disabling symptoms despite injections and conservative treatment and ultimately benefit from surgical decompression.

A 2025 systematic review of 72 randomized trials involving more than 7,700 patients with lumbar disc herniation found that epidural steroid injections improved outcomes compared with some conservative approaches, particularly in the short term, while surgery tended to provide more immediate and durable relief in appropriately selected patients.

The decision should therefore be individualized.


What If the First Epidural Injection Doesn’t Work?

This is where diagnosis becomes extremely important.

If an appropriately performed injection produces no meaningful benefit, there are several possibilities.

The diagnosis may be incorrect

The pain may not actually be coming from a compressed spinal nerve.

Other conditions can mimic sciatica, including:

  • Hip disorders
  • Sacroiliac joint pain
  • Peripheral nerve disorders
  • Piriformis syndrome
  • Muscular conditions
  • Vascular conditions

The wrong level may have been targeted

The MRI abnormality may not correspond to the nerve producing the symptoms.

The pathology may be too severe

A severely compressed nerve may not respond adequately to an injection.

The pain may not be primarily inflammatory

If the dominant problem is mechanical compression rather than inflammation, an injection may have limited benefit.

The injection may simply not have worked

Even when the diagnosis and technique are appropriate, no treatment works for every patient.


Should You Have Another Epidural Injection If the First One Helped?

Sometimes.

If an injection provides meaningful but temporary relief and the symptoms later return, repeating the procedure may be reasonable in selected patients.

However, epidural injections should not be performed indefinitely simply because they provide temporary relief.

The overall treatment plan should consider:

  • How much relief the first injection provided
  • How long the relief lasted
  • Whether function improved
  • Whether the underlying condition is changing
  • Whether physical therapy or other treatment is addressing contributing factors
  • Whether another treatment or surgical consultation is more appropriate

The goal should be meaningful improvement in pain and function, not simply performing injections on a predetermined schedule.


How Many Epidural Steroid Injections Can You Have?

There is no universal number that is appropriate for every patient.

The frequency and total number of injections should be individualized based on clinical response, the underlying condition, medication exposure, and other patient-specific factors.

Repeated steroid exposure can have systemic effects, even when the medication is injected into the spine.

For that reason, epidural injections should generally be used as part of a broader treatment plan rather than as a routine, indefinite treatment.


What Are the Risks of an Epidural Steroid Injection?

Epidural steroid injections are commonly performed procedures, but they are not risk-free.

Potential complications include:

  • Temporary increase in pain
  • Headache
  • Bleeding
  • Infection
  • Nerve injury
  • Dural puncture
  • Spinal fluid leak
  • Temporary numbness or weakness
  • Elevated blood glucose
  • Steroid-related side effects
  • Allergic or adverse reactions

Rare but serious neurological complications have also been reported.

The FDA has warned about rare serious neurologic events associated with epidural corticosteroid injections, including spinal cord infarction, paralysis, stroke, vision loss, and death. The FDA also notes that corticosteroids are not FDA-approved specifically for epidural administration, meaning this use is off-label.

These complications are uncommon, but they are important reasons why epidural injections should be performed by appropriately trained clinicians using appropriate technique and imaging guidance.


Are Steroid Injections Safe for Everyone?

No.

Your physician should review your medical history before performing an epidural injection.

Important considerations include:

  • Blood-thinning medications
  • Bleeding disorders
  • Infection
  • Diabetes
  • Immune suppression
  • Steroid intolerance
  • Allergies to contrast or medications
  • Pregnancy
  • Certain anatomical abnormalities
  • Previous spine surgery

Medication management, particularly with anticoagulants and antiplatelet medications, requires individualized assessment.

Patients should never stop a blood thinner on their own simply because they are planning to have an injection.


Does the Type of Steroid Matter?

Yes.

Corticosteroids used for epidural injections can be broadly categorized as particulate or non-particulate.

Examples of particulate corticosteroids include:

  • Methylprednisolone
  • Triamcinolone
  • Betamethasone preparations containing particulate components

Dexamethasone is commonly used as a non-particulate corticosteroid.

The choice of steroid can depend on the procedure, anatomy, risk considerations, physician preference, and applicable safety recommendations.

Particular attention has been paid to the safety of steroid selection during transforaminal injections because of the rare but devastating neurological complications associated with inadvertent arterial injection.

This is another reason that technique, imaging, contrast confirmation, and appropriate medication selection matter.


What Is the Difference Between an Epidural Steroid Injection and a Selective Nerve Root Block?

The terminology can sometimes be confusing.

A selective nerve root block (SNRB) is designed to target a specific spinal nerve root.

An injection performed through the transforaminal approach may function therapeutically as an epidural injection while also providing diagnostic information about a particular nerve root.

The exact terminology can vary depending on the clinical context and how the procedure is being used.

If your physician recommends a “nerve root injection,” it is reasonable to ask:

“Are we treating the nerve therapeutically, using the injection diagnostically, or both?”

That question can help clarify the purpose of the procedure.


What Is the Difference Between an Epidural Steroid Injection and a Facet Joint Injection?

These procedures treat different pain generators.

Epidural steroid injection

Generally used for symptoms related to irritation of a spinal nerve root, such as:

  • Sciatica
  • Lumbar radiculopathy
  • Arm pain from cervical radiculopathy

Facet joint injection

Targets the facet joints of the spine and is generally used for pain thought to originate from those joints.

Facet-mediated pain is typically axial back pain, rather than classic shooting pain down the leg in a nerve-root distribution.

This distinction is important.

If you have classic sciatica from a herniated disc, a facet injection would generally not be the first injection you would consider.


What Is the Difference Between an Epidural Steroid Injection and Radiofrequency Ablation?

These procedures are also designed for different pain generators.

Epidural steroid injections are generally used for radicular pain associated with spinal nerve irritation.

Radiofrequency ablation (RFA) is typically used to treat pain arising from the medial branch nerves that supply the facet joints.

RFA is therefore generally a treatment for facet-mediated axial back or neck pain, not classic sciatica caused by a herniated disc.

This distinction is important because patients sometimes hear about “nerve ablation” and assume it is another treatment for sciatic nerve pain.

Who Is a Good Candidate for an Epidural Steroid Injection?

You may be a reasonable candidate if you have:

  • Leg pain consistent with lumbar radiculopathy
  • Symptoms that correlate with a particular spinal nerve
  • Imaging showing a potentially relevant disc herniation or stenosis
  • Persistent or severe symptoms despite appropriate conservative treatment
  • Pain that is limiting your ability to function or participate in rehabilitation

The decision should be based on the whole clinical picture, not the MRI alone.


Who May Not Benefit From an Epidural Injection?

An epidural injection is less likely to help when the primary problem is:

  • Non-specific axial low back pain
  • Facet-mediated pain
  • Hip arthritis
  • Sacroiliac joint pain
  • Peripheral neuropathy
  • A muscular condition
  • Another non-spinal source of leg pain

This is why determining the pain generator is so important before performing an injection.


Does Everyone With a Herniated Disc Need an Epidural Injection?

Absolutely not.

A herniated disc on an MRI does not automatically mean that an injection is necessary.

Some people have large disc herniations and minimal symptoms.

Others have relatively small abnormalities that produce severe radicular pain.

The important question is:

Does the disc herniation actually correlate with the patient’s symptoms and examination?

This is one of the most important principles in spine care.


What If My MRI Shows a Herniated Disc But I Don’t Have Leg Pain?

In that situation, an epidural steroid injection may not be appropriate.

If your only symptom is low back pain, your physician should consider other potential pain generators before recommending an epidural injection.


Can an Epidural Injection Help Spinal Stenosis?

It depends on the symptoms.

Spinal stenosis can produce several different clinical syndromes.

A patient with stenosis and radicular leg pain may potentially benefit from an epidural injection.

However, a patient whose primary problem is severe neurogenic claudication—leg pain or heaviness that occurs with standing and walking—may respond differently.

The 2025 AAN evidence review found that epidural steroid injections may modestly improve disability in lumbar spinal stenosis but did not demonstrate meaningful short-term pain reduction, highlighting the importance of distinguishing different stenosis-related symptom patterns.


What Should You Do After an Epidural Injection?

The answer depends on your symptoms and the reason the injection was performed.

In many cases, the injection is most useful when combined with an active rehabilitation program.

As pain improves, patients may be able to:

  • Walk more

  • Address contributing movement or conditioning issues

An injection should ideally be viewed as one component of a treatment plan, rather than the entire treatment.


The Bottom Line: What Is the Best Injection for Sciatica?

There is no single “best” injection for every person with sciatica.

For a patient with unilateral lumbar radicular pain caused by a disc herniation or focal nerve-root compression, a transforaminal epidural steroid injection may be an excellent option because it allows medication to be delivered near the affected nerve root.

An interlaminar injection may be more appropriate in other situations, while a caudal approach can be useful for selected patients with multilevel disease, previous surgery, or anatomy that makes another approach less desirable.

The most important question is not:

“Which injection is the strongest?”

It is:

“Does the patient’s diagnosis identify a pain generator that is likely to respond to an epidural injection, and which approach provides the most appropriate and safest access to that target?”

Epidural steroid injections can provide meaningful short-term relief for appropriately selected patients with lumbar radiculopathy, but they are not a permanent cure and do not eliminate the underlying structural cause of nerve compression. Current evidence supports their role primarily as a treatment for selected radicular pain, with the strongest evidence for short-term rather than guaranteed long-term benefit.
