Sciatica: Symptoms, Causes, Treatment & How to Get Relief
Sciatica is one of the most common causes of pain that travels from the lower back into the buttock and leg. It can cause sharp, burning, or electric-like pain, as well as numbness, tingling, or weakness.
The good news is that most people with sciatica improve without surgery. However, persistent or severe symptoms may require additional evaluation and treatment, including physical therapy, medications, epidural steroid injections, or, in selected cases, surgery.
As a physician specializing in interventional spine and musculoskeletal medicine, I frequently evaluate patients who come to me with severe leg pain and wonder whether they have “sciatica,” whether they need an MRI, and whether an injection or surgery is necessary.
One of the most important things I tell patients is that sciatica is not simply a diagnosis based on where you hurt. The pattern of your symptoms, physical examination, and—when appropriate—imaging all help determine whether a spinal nerve is actually being irritated or compressed.
This guide explains what sciatica is, what causes it, how it is diagnosed, how long it usually lasts, and the treatment options available.
What Is Sciatica?
Sciatica refers to pain and other symptoms caused by irritation or compression of one or more of the nerve roots in the lower spine that contribute to the sciatic nerve.
The sciatic nerve is formed by nerve roots originating in the lower lumbar and sacral spine. It travels from the lower back through the buttocks and into the leg.
Although people commonly describe the problem as a “pinched sciatic nerve,” the actual problem usually occurs before the sciatic nerve is formed, at one of the nerve roots as it exits the spine.
The most common causes include:
- Herniated or ruptured discs
- Degenerative changes in the spine
- Bone spurs
- Narrowing of the spinal canal or neural foramen
- Spondylolisthesis
- Less commonly, other conditions that compress or irritate a spinal nerve
A herniated disc can cause sciatica when disc material protrudes far enough to irritate or compress a nearby nerve root. Spinal degeneration can also cause narrowing around the nerves and produce similar symptoms.
Sciatica vs. lumbar radiculopathy
The terms sciatica and lumbar radiculopathy are sometimes used interchangeably, but they are not exactly the same.
Sciatica describes a characteristic pattern of pain and other symptoms traveling into the leg.
Lumbar radiculopathy is a medical term describing dysfunction of a lumbar or sacral nerve root, which may cause pain, numbness, sensory changes, or weakness.
In other words, sciatica describes a symptom pattern, while lumbar radiculopathy describes a nerve-root problem.
What Does Sciatica Feel Like?
Sciatica can feel very different from ordinary low back pain.
Common descriptions include:
- Sharp or stabbing pain
- Burning pain
- Electric or shock-like pain
- Shooting pain traveling down the leg
- Deep aching pain in the buttock
- Numbness
- Tingling or “pins and needles”
- Weakness in the leg or foot
Sciatic pain commonly travels from the lower back or buttock into the thigh, calf, or foot. It typically affects one side, although symptoms can occur on both sides depending on the underlying cause.
Some people have severe leg pain with relatively little back pain.
This is an important distinction because severe pain in the leg does not necessarily mean that the problem is primarily in the leg. A compressed nerve in the lower spine can produce pain far away from the actual source of the problem.
Where does sciatica hurt?
The exact location depends on which nerve root is affected.
For example, irritation of different lumbar nerve roots can produce symptoms in different portions of the thigh, calf, ankle, or foot.
This distribution is one of the reasons that a careful neurological examination can be so helpful.
What Causes Sciatica?
1. Herniated Disc
A herniated disc is one of the most common causes of acute sciatica.
The discs between the vertebrae act as cushions between the bones of the spine. A disc can develop a tear or weakness in its outer portion, allowing the inner material to protrude.
If the disc herniation comes into contact with a nearby nerve root, it can cause inflammation and/or mechanical compression of the nerve.
Interestingly, not every disc herniation causes pain.
Many people have disc bulges or herniations on MRI scans without having any symptoms. This is why an MRI finding must be interpreted in the context of the patient’s symptoms and physical examination.
2. Spinal Stenosis
Spinal stenosis occurs when the spaces available for the spinal nerves become narrowed.
This can happen because of:
- Bone spurs
- Enlarged facet joints
- Thickened ligaments
- Disc degeneration
- Disc bulging
- Spondylolisthesis
Spinal stenosis becomes more common with aging.
Unlike the sudden onset of sciatica caused by some disc herniations, symptoms from stenosis often develop gradually.
Patients may experience pain, numbness, heaviness, or weakness in the legs when standing or walking.
A classic symptom pattern is neurogenic claudication—leg symptoms that worsen with standing or walking and improve with sitting or bending forward.
3. Degenerative Disc Disease
As we age, the discs in the spine naturally undergo structural changes.
The discs can lose height and flexibility, which can reduce the space available for nearby nerves.
Degenerative disc disease itself does not necessarily cause sciatica. However, the changes associated with disc degeneration can contribute to foraminal narrowing or other forms of nerve compression.
4. Bone Spurs
Arthritis and degenerative changes in the spine can produce excess bone growth called osteophytes, or bone spurs.
If a bone spur narrows the space around a nerve, it can contribute to radicular symptoms.
5. Spondylolisthesis
Spondylolisthesis occurs when one vertebra shifts relative to another.
Depending on the severity and location, this can contribute to narrowing around the spinal nerves and cause symptoms in the back or legs.
6. Less Common Causes
Less commonly, symptoms resembling sciatica can result from:
- Spinal cysts
- Tumors
- Infection
- Fractures
- Other disorders affecting the nerves or spine
These causes are uncommon, but certain symptoms or medical histories may warrant more urgent investigation.
Who Is at Risk for Sciatica?
Sciatica can affect people of almost any age, but several factors can increase the likelihood of developing it.
These include:
- Increasing age
- Physically demanding work
- Repetitive lifting or twisting
- Prolonged sitting
- Obesity
- Diabetes
- Previous episodes of back pain or sciatica
- Activities that place significant stress on the lower back
However, sciatica can occur even in people who have none of these risk factors.
Sciatica vs. Other Causes of Leg Pain
Not every pain traveling into the leg is sciatica.
Several conditions can produce similar symptoms, including:
- Hip arthritis
- Sacroiliac joint pain
- Piriformis syndrome
- Peripheral neuropathy
- Muscle or tendon injuries
- Hamstring problems
- Vascular disease
- Other peripheral nerve disorders
This is why simply having pain in the buttock or leg does not automatically mean that a spinal nerve is being compressed.
The location and character of the pain, neurological findings, provocative maneuvers, and sometimes imaging help distinguish these conditions.
How Is Sciatica Diagnosed?
The diagnosis of sciatica usually begins with a detailed medical history and physical examination.
Your physician will typically want to know:
- Where the pain starts
- Where the pain travels
- Whether you have numbness or tingling
- Whether you have weakness
- What positions make the symptoms better or worse
- Whether coughing or sneezing increases the pain
- How long the symptoms have been present
- Whether there was an injury
- Whether you have had similar symptoms previously
The neurological examination
A neurological examination can provide important information about which nerve may be affected.
Your physician may evaluate:
- Muscle strength
- Reflexes
- Sensation
- Walking
- Heel and toe walking
- Straight-leg raise or other nerve-tension tests
The findings can help determine whether the symptoms are consistent with a specific nerve-root problem.
Do You Need an MRI for Sciatica?
Not everyone with sciatica needs an MRI.
This is one of the most common misconceptions I encounter.
For many patients with a typical presentation of acute sciatica and no concerning neurological findings or red flags, symptoms can improve without immediate advanced imaging.
An MRI becomes more useful when:
- Symptoms are severe or persistent
- Symptoms are not improving as expected
- There is significant or progressive weakness
- The diagnosis is uncertain
- An injection or surgical procedure is being considered
- There are concerning symptoms suggesting a more serious condition
MRI is particularly useful because it can show the discs, spinal canal, neural foramina, and nerve-root compression.
A word of caution about MRI findings
An MRI can show abnormalities that are completely unrelated to your symptoms.
Disc bulges, disc degeneration, arthritis, and even disc herniations can occur in people who have no pain.
Therefore, the goal isn’t simply to find an abnormality on MRI. The goal is to determine whether the abnormality explains the patient’s symptoms.
This is one of the most important concepts in evaluating spine pain.
Do You Need an EMG or Nerve Conduction Study?
Sometimes.
Electromyography (EMG) and nerve conduction studies can provide additional information about nerve function.
They may be useful when:
- The diagnosis is uncertain
- Symptoms do not clearly match the MRI
- Peripheral neuropathy is being considered
- A peripheral nerve problem needs to be distinguished from a spinal nerve-root problem
- Symptoms have persisted for an appropriate period of time
An EMG does not replace a good history and physical examination, and it is not necessary for every patient with sciatica.
How Long Does Sciatica Last?
The natural history of sciatica varies depending on its underlying cause.
Many episodes—particularly those associated with a disc herniation—improve substantially over several weeks to a few months. Most patients do not ultimately require surgery.
The course can be influenced by:
- The cause of the nerve irritation
- Severity of the nerve compression
- Degree of inflammation
- Presence of neurological deficits
- Overall health
- Activity level
- Response to treatment
It’s important to understand that pain often improves before numbness or weakness fully resolves.
Nerves can take considerably longer to recover than the surrounding tissues.
Treatment for Sciatica
The appropriate treatment depends on the cause and severity of the symptoms.
There is no single “best treatment” for every patient.
Treatment may include a combination of:
- Activity modification
- Physical therapy
- Medications
- Home exercises
- Epidural steroid injections
- Other targeted injections
- Surgery in selected cases
1. Staying Active
One of the biggest mistakes people make when they develop severe sciatica is assuming they need to stay in bed.
A brief period of relative rest may be helpful during a severe flare, but prolonged inactivity can lead to deconditioning and may make recovery more difficult.
In general, patients should try to remain as active as their symptoms allow while avoiding activities that significantly aggravate their pain.
Walking is often a good starting point.
The goal is not to “push through” severe nerve pain. Instead, gradually return to normal movement as symptoms improve.
2. Physical Therapy
Physical therapy is an important component of treatment for many patients with sciatica.
A physical therapist can help with:
- Mobility
- Core and trunk strengthening
- Hip strength
- Flexibility
- Movement mechanics
- Gradual return to activity
- Education about positions and activities that aggravate symptoms
The exercises should be individualized.
An exercise that helps one patient may aggravate another, particularly when the underlying causes of their symptoms are different.
3. Medications
Depending on the individual patient, medications may be used to help control pain while the underlying condition improves.
Potential medications include:
- Nonsteroidal anti-inflammatory drugs (NSAIDs)
- Acetaminophen
- Selected medications for neuropathic pain
- Short-term muscle relaxants in appropriate situations
- Other medications when clinically indicated
Medication selection should take into account the patient’s other medical conditions, kidney and gastrointestinal health, cardiovascular risk, medication interactions, and potential side effects.
No medication is appropriate for everyone.
4. Epidural Steroid Injections
An epidural steroid injection can be considered for selected patients with persistent or severe radicular pain.
The goal is to place an anti-inflammatory medication near the irritated spinal nerve.
There are several approaches, including:
- Transforaminal epidural steroid injection
- Interlaminar epidural steroid injection
- Caudal epidural steroid injection
The approach selected depends on the patient’s anatomy, symptoms, imaging findings, and clinical goals.
Do epidural injections work?
For appropriately selected patients with lumbar radiculopathy, epidural steroid injections can provide short-term pain relief, although they do not permanently eliminate the underlying structural problem. Evidence reviewed by the American Academy of Neurology found a probable short-term reduction in pain for cervical and lumbar radiculopathy.
I often describe an epidural injection to patients as a way of calming the irritated nerve enough to allow the patient to participate more effectively in rehabilitation and return to normal activity.
An injection is not necessarily a cure, and it is not appropriate for every patient.
5. When Is Surgery Necessary?
Fortunately, most people with sciatica do not need surgery.
Surgery may become appropriate when:
- There is significant or progressive neurological weakness
- Symptoms are severe and persistent despite appropriate nonsurgical treatment
- Imaging demonstrates a structural problem that matches the patient’s symptoms
- There is significant functional impairment
- There is an emergency involving bowel or bladder dysfunction
For patients with a disc herniation causing persistent nerve compression, surgery may involve removing the portion of the disc that is pressing on the nerve.
The decision to proceed with surgery should be individualized and based on the combination of symptoms, neurological findings, imaging, duration of symptoms, and response to nonsurgical treatment.
When Is Sciatica an Emergency?
Most cases of sciatica are not emergencies.
However, certain symptoms require immediate medical evaluation.
Seek emergency medical attention if you develop:
- New loss of bladder control
- New loss of bowel control
- Difficulty initiating urination
- Numbness around the groin, genitals, or inner thighs
- Severe or rapidly progressive weakness in the legs
- Significant weakness affecting both legs
These symptoms can indicate cauda equina syndrome, a rare but potentially serious condition requiring urgent evaluation.
You should also seek prompt medical evaluation after significant trauma or when back and leg pain is accompanied by concerning systemic symptoms such as fever, unexplained weight loss, or a history of cancer.
Can Sciatica Come Back?
Yes.
Unfortunately, having one episode of sciatica does not make you immune from developing another.
Recurrence can occur because the underlying disc degeneration, spinal stenosis, or other structural changes may remain even after the initial episode has resolved.
The good news is that maintaining regular physical activity, improving strength and conditioning, addressing modifiable risk factors, and learning how to manage flare-ups can help reduce the impact of future episodes.
How Can You Prevent Sciatica?
There is no guaranteed way to prevent sciatica.
However, several habits may help protect your spine:
Exercise regularly
Regular aerobic exercise and strength training can help maintain overall conditioning and support the muscles around your spine.
Maintain a healthy body weight
Excess body weight can increase mechanical stress on the spine.
Avoid prolonged inactivity
Long periods of sitting or inactivity can contribute to deconditioning.
Use good lifting mechanics
When lifting heavier objects:
- Keep the object close to your body
- Bend through your hips and knees
- Avoid twisting while lifting
- Ask for help with particularly heavy or awkward objects
Gradually increase activity
A sudden increase in lifting, exercise volume, or other physical activity can trigger symptoms in some people.
Gradual progression is generally preferable to going from relatively inactive to extremely active overnight.
What About Stretching for Sciatica?
Stretching can be helpful for some people, but there is no single stretch that fixes sciatica.
The appropriate exercises depend on the cause of the symptoms and how the individual responds to different movements.
For some patients, certain positions can reduce their symptoms. For others, the same movement may make their leg pain significantly worse.
If an exercise causes your pain to travel farther down the leg, produces new numbness or weakness, or substantially increases your symptoms, stop and discuss it with a qualified healthcare professional.
What About Chiropractic Treatment, Massage, or Acupuncture?
Some patients with back and leg pain benefit from complementary treatments such as massage, acupuncture, or spinal manipulation.
However, the appropriate treatment depends heavily on the underlying diagnosis.
These approaches should not delay evaluation when a patient has significant neurological symptoms or other red flags.
The most important first step is determining what is actually causing the symptoms.
Common Questions About Sciatica
Is sciatica the same as a pinched nerve?
Not exactly.
Sciatica is a symptom pattern involving pain or other symptoms traveling along the distribution of the sciatic nerve. A pinched or irritated lumbar nerve root is one of the most common mechanisms responsible for sciatica.
Can you have sciatica without back pain?
Yes.
Some patients have relatively little low back pain and experience most of their symptoms in the buttock and leg.
Can sciatica cause numbness?
Yes.
Numbness and tingling are common symptoms of nerve irritation or compression.
Can sciatica cause weakness?
Yes.
Significant weakness is more concerning than pain alone and warrants a careful neurological examination.
Progressive or severe weakness requires prompt medical evaluation.
Is walking good for sciatica?
Often, yes.
Walking is generally preferable to prolonged bed rest, provided it does not substantially worsen your symptoms.
However, patients with severe spinal stenosis may find that walking aggravates their symptoms while sitting or leaning forward provides relief.
Is sitting bad for sciatica?
Sitting can aggravate symptoms in some people, particularly those with certain types of disc-related nerve irritation.
However, not everyone with sciatica experiences the same pattern.
Your response to different positions can actually provide your physician with useful diagnostic information.
Should I use heat or ice for sciatica?
Either may provide temporary symptom relief.
Some people prefer ice, while others find heat more comfortable. There is no requirement that every patient use one rather than the other.
The most important consideration is whether it helps your symptoms.
Does sciatica always require an MRI?
No.
Many patients can initially be diagnosed clinically without advanced imaging.
MRI becomes more useful when symptoms persist, neurological deficits develop, the diagnosis is uncertain, or an invasive treatment or surgery is being considered.
Does a herniated disc always cause sciatica?
No.
Many people have disc bulges or herniations without symptoms.
An imaging abnormality is meaningful only when it correlates with the patient’s symptoms and examination.
Do epidural steroid injections cure sciatica?
Not necessarily.
An epidural steroid injection is intended primarily to reduce inflammation and pain around an irritated nerve. It may provide meaningful relief for an appropriate patient, but it does not necessarily correct the underlying disc herniation or spinal stenosis.
Does sciatica require surgery?
Usually not.
Most people improve without surgery.
Surgery becomes more relevant when there is significant or progressive neurological deficit, persistent disabling symptoms despite appropriate conservative treatment, or an emergency condition such as cauda equina syndrome.
How long should I wait before seeing a doctor?
If your symptoms are mild and improving, you may be able to initially manage them with activity modification and other conservative measures.
You should seek medical evaluation if the pain is severe, worsening, persistent, interfering substantially with your daily activities, or associated with numbness or weakness.
Immediate evaluation is necessary for new bowel or bladder dysfunction, saddle numbness, or significant/progressive weakness.
The Bottom Line
Sciatica is a common cause of pain, numbness, tingling, and sometimes weakness traveling from the lower back or buttock into the leg.
The most common causes include herniated discs and degenerative narrowing around the spinal nerves.
The good news is that most patients improve with time and nonsurgical treatment. Physical therapy, activity modification, medications, and—in selected patients—epidural steroid injections can help manage symptoms while the irritated nerve recovers.
However, persistent or severe symptoms should not simply be ignored.
A careful history and neurological examination can help determine whether your symptoms truly represent sciatica, while MRI or other diagnostic testing may be appropriate in selected circumstances.
Most importantly, an MRI abnormality does not automatically explain your pain. The best treatment plan is one that connects your symptoms and physical examination with the structural findings on imaging.
